HIPAA / PHI Handling
HPC Testing Services may receive protected health information (PHI) from hospitals, clinics, or other healthcare providers as part of drug/alcohol testing, DNA testing, and chain-of-custody processes. While we may not be a traditional HIPAA covered entity, we handle all PHI according to HIPAA security standards where applicable:
* Staff Training: Personnel handling PHI are trained in proper handling, faxing, emailing, and storage practices to ensure confidentiality.
* Physical Security: Paper records and sensitive documents are securely stored in locked cabinets with restricted access. Shredding is performed when information is no longer required.
* Transmission of Results: Drug and alcohol test results and DNA testing results are transmitted only via secure email or verified fax to authorized recipients, or provided in-person.

HPC Testing Services may contact clients via text messaging at the phone number provided for:
* Appointment reminders
* General reminders regarding the expiration of your medical certificate/exam
* Providing operational information such as our hours and location
By providing a cell phone number during sign-up, you consent to receiving these types of messages at the number provided.
* This service is provided at no cost by HPC Testing Services, but standard text messaging rates may apply according to your wireless plan.
* If you no longer wish to receive text messages from HPC Testing Services, you may opt out by notifying us in writing or replying to the text message with instructions to stop.

Payments and Refunds
* All online payments are securely processed through Stripe via the InvoiceNinja portal.
* Payments are final. Refunds are only available in the case of billing errors and must be requested within 30 days of the transaction. All refund requests are subject to approval by HPC Testing Services.
Client Responsibilities
* Clients must provide accurate and complete information when requesting services or making payments.
Limitations of Liability
* HPC Testing Services acts solely as a collection site and does not perform laboratory testing.
* All testing is conducted by approved, certified laboratories.
* We are not responsible for the results, errors, or timing of testing performed by third-party labs.
* HPC Testing Services provides guidance and management for compliance and testing, but cannot guarantee employer or regulatory outcomes.
* We are not liable for injuries, accidents, or incidents that may occur outside of our collection site during testing.
* Any advice or compliance information provided is informational only and should not be considered legal, medical, or regulatory advice.
* HPC Testing Services is also not responsible for errors or downtime caused by third-party systems such as Stripe or InvoiceNinja.
